Make Appointment with Tom De Pauw

To make an appointment please call our rooms on 0457268356. In order to prioritise appointments appropriately Tom De Pauw will triage all referrals. Please ensure your referral from your General Practitioner is faxed or brought to the rooms in order to expedite this process for you.

What to bring to your first appointment...

• The referral from your GP
• Any relevant reports or letters
• Any history of previous medications/treatments trialed

Please be aware that completion of reports and forms outside of appointments will incur a fee.

Tom De Pauw bases his fee structure on the APS recommended fee schedule for appointments and QCOMP/ Workcover recommended fee schedules for most reports.

Please arrive to your first appointment approximately 15 minutes early in order to complete relevant paperwork. Parking may be difficult however can usually be found on North Street itself or in the designated car park at the front of the building.

Please confirm appointments 24 hours prior to avoid cancellation fees.
